LEVELS OF CARE (LOC)
Residential Program placement and rates are based on the client’s degree of need and amount of services necessary to meet those needs. A Level of Care (LOC) evaluation is performed prior to admission and periodically thereafter to determine the degree of services needed.
Factors taken into consideration when determining the client’s LOC are:
- Functional status, or the ability to manage daily routines;
- Medical/health condition, and whether chronic or severe;
- Behavior, and the need for monitoring and/or management
- Frequency of contact by licensed professionals
- Senior Day Care
- Overnight Respite Care
- Skilled Nursing (SNF) and Intermediate Care (ICF)
- Physical, Occupational, and Speech Therapies
- Adult Residential Care Home (ARCH Type II)
- Hospice Services
