LEVELS OF CARE (LOC)

Residential Program placement and rates are based on the client’s degree of need and amount of services necessary to meet those needs. A Level of Care (LOC) evaluation is performed prior to admission and periodically thereafter to determine the degree of services needed.

Factors taken into consideration when determining the client’s LOC are:

  • Functional status, or the ability to manage daily routines;
  • Medical/health condition, and whether chronic or severe;
  • Behavior, and the need for monitoring and/or management
  • Frequency of contact by licensed professionals

CARE PROGRAMS

 

Senior Day Care

Palolo Chinese Home’s Senior Day Care program enables seniors to continue living at home with family and attend a day program that provides supervised care, stimulating activities and exercise, peer socialization, and nutritious meals. Licensed for 40 participants, PCH Senior Day Care program hours are 7:30 a.m. to 5:30 p.m., seven days a week, including holidays.

Overnight Respite Care

"Respite" refers to short term, temporary care provided to people with disabilities in order that their families can take a break from the daily routine of care giving. Palolo Chinese Home’s Overnight Respite Care program care enables families to take care of business or go on vacation with peace of mind that their loved one is receiving care in a supervised and structured care setting. Regular or periodic stays lasting from a few days to several weeks can be arranged in the PCH’s Adult Residential Care Home. Overnight Respite residents enjoy all the benefits of our ARCH Type II programs.

Skilled Nursing (SNF) and Intermediate Care (ICF)

When a person can no longer remain at home because of a chronic health condition, Palolo Chinese Home’s trained professionals are ready to provide 24 hour care specific to each individual. PCH is licensed by the State of Hawaii and certified by Medicare and Medicaid to provide intermediate and skilled nursing residential services. An entire team of nurses, therapists and other health professionals develop Individually designed care plans to maximize each resident’s potential and increase quality of life. PCH is currently licensed for 61 ICF/SNF beds.   Medicare and Medicaid are accepted.

Physical, Occupational, and Speech Therapies

Persons who need nursing home care to recover from a disabling injury or illness, will have a plan of care that will include rehabilitation services for strength and balance to be independent again. Brighton Rehabilitation, contract provider of rehabilitation therapy at Palolo Chinese Home, has been delivering quality rehabilitation services throughout the Western United States for more than 25 years. The PCH Wellness Center provides therapeutic rehabilitation services to PCH residents, day care clients and community members under the direction of the of their physicians and provided by qualified professional therapists with the aim of returning participants to the highest level of function possible. Participation in the Wellness Therapy Program at PCH by community members requires a physician’s order and is covered by insurance such as Medicare Outpatient B.

Adult Residential Care Home (ARCH Type II)

As one of the oldest institutions of its kind in the nation, Palolo Chinese Home continues to be a leader in providing quality, affordable, home-like care at our beautiful 15-acre campus. Services include room and board, assistance with activities of daily living, medical oversight, as well as daily social, cultural and restorative activities. PCH is currently licensed to serve 50 residents. Expanded Care, Dementia Care and Diabetes Care are available.

Hospice Services

Palolo Chinese Home partners with several hospice service organizations to provide humane and compassionate care for our residents so that they may live with quality and dignity, and as comfortable as possible. A team of professionals, including nurses, certified nurse aides, social worker, and chaplain, provide care for the patient and family that focuses on quality of life. The services are generally covered by Medicare, Medicaid and several private insurance plans. Medications, medical equipment (e.g. oxygen) and medical supplies related to the terminal illness are also generally covered. If your health plan has hospice residential benefit coverage, room and board nursing facility charges may also be covered.

As an only child with two young children and aging parents to care for, I am fortunate to have found the Palolo Chinese Home Adult Day Care program.  Being a full-time caregiver to young children and elderly parents has been challenging and often exhausting. I appreciate that the day care is open seven days a week as it has allowed me to do fun, educational and recreational activities with my children, all the while knowing that my parents are well cared for.  The facilities at Palolo are clean and safe, and the staff members are cheerful and genuinely care about my parents.  When my father passed away last year, they were kind enough to reminisce with me and shared their own memories of him -- it made me feel as though they thought of him as part of their family too. My mother, who suffers from Alzheimer's, continues to attend the program daily.  She has found comfort in the routine and feels safe with the staff. They have supported her since my father passed away and even play hanafuda with her -- something that she looks forward to!
